Sonic & All-Stars Racing Transformed is a mascot-based racing game in which players race against each other using characters from various Sega franchises, although it features many new mechanics and improvements from the last game. The most obvious is that, at certain points in a race, vehicles can alternate between car, boat and plane modes. The car mode handles similarly to the previous game, in which players can earn boost by drifting around corners or performing tricks whilst in the air.

Most tracks are based off various locations in the Mario series, but two tracks are set on Wuhu Island, which was featured in non-Mario video games Wii Fit, Wii Fit Plus and Wii Sports Resort. While driving, the players use power-up items, obtained from running into item boxes located at specific points on the track, that are used to hinder the progression of opponents or to help the player in the race.

The 3DS version also features Autolog, which is integrated throughout the career mode and uses 40 in-game challenges scattered throughout the campaign. The game also takes advantage of StreetPass, letting players upload their best Autolog scores to other Nintendo 3DS devices. In multiplayer, the game features a straight race mode where eight people compete. There is also a four-on-four Cops vs. Racers mode. The game supports Wi-Fi and local wireless connections. However, the Wii version lacks online play, yet it has split-screen multiplayer.
